Registered-Nurse Medication Support
Our medication management services are delivered by registered nurses to help you take your medications safely, correctly, and on time. Giving the medication, setting up the dose aid, reviewing what you take and watching for side effects. It reduces the risks that come with complex medication regimes.
As a clinical support under the Support at Home Programme, medication management is fully government-funded — there is no participant contribution. We coordinate closely with your GP and pharmacy to keep your medications accurate, safe, and continuously supplied across Geelong, the Bellarine Peninsula, and the Surf Coast.

Medication Administration
Professional administration of prescribed medications at the right time, in the right dose, through the right route, ensuring medication compliance and safety.
Medication Reviews
A review of everything you take, prescription and over-the-counter, checking for interactions and anything no longer needed, with a written report to your doctor.
Medication Organisation
Setup and maintenance of dose administration aids such as Webster packs, blister packs, and automated dispensers to simplify medication management.
Medication Education
Clear explanations about your medications including purpose, proper use, potential side effects, and important monitoring requirements to promote medication literacy.
Side Effect Monitoring
Regular assessment for medication side effects with proactive interventions and coordination with your prescribing doctor to adjust treatments as needed.
Medication Tracking
Keeping your medication records straight, managing repeats, and dealing with the pharmacy so you do not run out.
