Nurse-Led Management of Ongoing Conditions
Our registered nurses help you manage ongoing health conditions — including diabetes, heart and respiratory conditions — through regular monitoring, medication management, symptom recognition, and personalised self-management education. The goal is stability, fewer hospital admissions, and confidence living well at home.
As a clinical support under the Support at Home Programme, chronic disease management is fully government-funded — there is no participant contribution. We coordinate with your GP and specialists to keep your care plan current across Geelong and surrounds.
Our Chronic Disease Management Services
Diabetes Management
Blood glucose monitoring, insulin, foot care, and help with diet — the day-to-day of managing diabetes at home.
Cardiac Care
Specialised management of heart conditions including monitoring vital signs, medication management, lifestyle modifications, and symptom recognition.
Respiratory Support
Expert care for chronic respiratory conditions such as COPD and asthma, including breathing techniques, inhaler management, and exacerbation prevention.
Medication Optimisation
A nurse reviews complex medication regimes across several conditions, looking for what is working, what is duplicated, and what may be causing side effects.
Health Monitoring
Regular monitoring of vital signs, symptoms, and health indicators to track condition stability, identify trends, and detect changes requiring intervention.
Education & Self-Management
Personalised education programs to develop self-management skills, condition understanding, symptom recognition, and when to seek medical assistance.
